In case of temporary and permanent disability of the insured or diagnosis of a “serious illness”
1. Application for payment of amounts and indemnities /according to a form/
2. Identity document (ID card, passport, etc.) of the insured person
3. Certificate for bank account of the insured person
4. Copy of medical certificates; Expert decisions of TEMC / NEMC; Epicrisis, etc.
5. When the event has occurred as a result of illness, documents certifying the initial diagnosis or clarification of the diagnosis, medical history and treatment are submitted – personal outpatient card, outpatient records, histological results, results of imaging and other tests, protocols of medical committees, prescription books and other official medical documents;
6. When the event has occurred as a result of an accident, documents containing information about the circumstances under which the event occurred are submitted – forensic examinations, examinations for the presence of alcohol in the blood, documents issued by the Ministry of the Interior and Traffic Police (traffic accident report, certificates, etc.), from the employer and the NSSI (declaration and protocol from the investigation of an occupational accident, order of the NSSI, etc.), from investigative and judicial bodies and the prosecutor’s office (court judgments, penal decrees, final decree of the preliminary proceedings, etc.), as well as official medical documents certifying the diagnosis (outpatient records, referrals, medical certificates, epicrisis, etc.).
Additional documents may be required for the payment of amounts in the event of a “serious illness”, depending on the specific case.
In case of hospital stay of the insured
1. Application for payment of amounts and indemnities /according to a form/
2. Identity document (ID card, passport)
3. Bank account certificate
4. A copy of the epicrisis from a hospital treatment
5. When the event has occurred as a result of illness, documents certifying the initial diagnosis or clarification of the diagnosis, medical history and treatment are submitted – personal outpatient card, medical certificates, outpatient records, prescription books, decisions of TEMC / NEMC and other official medical documents;
6. When the event has occurred as a result of an accident, documents containing information about the circumstances under which the event occurred are submitted – forensic examinations, documents issued by the Ministry of the Interior and Traffic Police (traffic accident report, certificates, etc.), from the employer and the NSSI (declaration and protocol from the investigation of an occupational accident, order of the NSSI, etc.), from investigative and judicial bodies and the prosecutor’s office (court judgments, penal decrees, final decree of the preliminary proceedings, etc.), as well as official medical documents certifying the diagnosis (outpatient records, referrals, medical certificates, epicrisis, etc.).
In case of reimbursement of medical expenses of the insured (expenses for medicines and supplies, etc.)

3. Certificate for bank account of the insured person
4. Documents certifying expenses incurred by the person – prescriptions, original invoices in the name of the person with the purchased medicines and supplies listed by type and quantity, accompanied by receipts, and other original payment documents.
5. When the event has occurred as a result of illness, documents certifying the initial diagnosis or clarification of the diagnosis, medical history and treatment are submitted – personal outpatient card, medical certificates, outpatient records, epicrisis, x-rays, protocols of medical committees, prescription books, decisions of TEMC / NEMC and other official medical documents;
6. When the event has occurred as a result of an accident, documents containing information about the circumstances under which the event occurred are submitted – forensic examinations, documents issued by the Ministry of the Interior and Traffic Police (traffic accident report, certificates, etc.), from the employer and the NSSI (declaration and protocol from the investigation of an occupational accident, order of the NSSI, etc.), from investigative and judicial bodies and the prosecutor’s office (court judgments, penal decrees, final decree of the preliminary proceedings, etc.), as well as official medical documents certifying the diagnosis (outpatient records, referrals, medical certificates, epicrisis, etc.).
In case of payments of amounts for surgical treatment

2. Identity document (ID card, passport)
3. Certificate for bank account of the insured person
4. A copy of the epicrisis from a hospital surgical treatment
5. When the event has occurred as a result of illness, documents certifying the initial diagnosis or clarification of the diagnosis, medical history and treatment are submitted – personal outpatient card, medical certificates, outpatient records, surgical records, results from imaging tests, decisions of TEMC / NEMC and other official medical documents;
6. When the event has occurred as a result of an accident, documents containing information about the circumstances under which the event occurred are submitted – forensic examinations, documents issued by the Ministry of the Interior and Traffic Police (traffic accident report, certificates, etc.), from the employer and the NSSI (declaration and protocol from the investigation of an occupational accident, order of the NSSI, etc.), from investigative and judicial bodies and the prosecutor’s office (court judgments, penal decrees, final decree of the preliminary proceedings, etc.), as well as official medical documents certifying the diagnosis (outpatient records, referrals, medical certificates, epicrisis, etc.).
7. When the event has occurred abroad and the issued documents are in a foreign language, a copy of the necessary documents is submitted. In some cases, submitting the original documents as well as an official legalized translation may be required.
In case of death of the insured person
1. Application for payment of amounts and indemnities /according to a form/
2. The first copy of the insurance policy with the general conditions attached to it, application for concluding the insurance, as well as the possibly issued lists and additional agreements /supplements/ thereto;
3. Identity document (ID card, passport) of the beneficiary, certificate of good standing, etc.;
4. Certificate for bank account of the beneficiary
5. Documents certifying the occurrence of the event – notarized copy of the death certificate or transcript-extract from the death certificate, death notice, medical documents (outpatient records, referrals, medical certificates, epicrisis, etc.);
6. When beneficiaries are not named in the policy but are the legal heirs of the insured, a notarized copy of the inheritance certificate is submitted
7. When death has occurred as a result of illness, documents certifying the initial diagnosis or clarification of the diagnosis, medical history and treatment are submitted – personal outpatient card, medical certificates, epicrises, decisions of TEMC / NEMC and other official medical documents;

When death has occurred as a result of an accident, documents containing information about the circumstances under which the event occurred are submitted – forensic examinations, examinations for the presence of alcohol in the blood, documents issued by the Ministry of the Interior and Traffic Police (traffic accident report, certificates, etc.), from the employer and the NSSI (declaration and protocol from the investigation of an occupational accident, order of the NSSI, etc.), from investigative and judicial bodies and the prosecutor’s office (court judgments, penal decrees, final decree of the preliminary proceedings, etc.), as well as other official documents, if issued, as well as other official medical documents certifying the diagnosis.
